PERSONAL RIGHTS. To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation... This requirement was not met as evicenced by: Based on interview with staff, a child was placed on a high chair to restrain him from climbing up and down the tables and chairs. Staff stated that this was done for "safety reasons".
Teacher Qualifications and Duties — Based on the interviews conducted with staff, it was determined that 2 aides were left to supervise preschool children for a few hours on different ocassions.
Buildings and Grounds — Based on interview with staff it was determined that a mice infestation occurred sometime around May or early June. Director did not seek professional exterminators to rid of the mice infestation.
Mandated reporter training — Based on record review the following staff have expired Mandated Reporter Training Certificate: Kendall Winn, Vira Virabutsady, Christina Alfonso and Andrea Clemente.
Personnel Requirements — Based on record review, staff Neri Saucedo did not have a current Physician's report when she was hired August 2023.
