Reporting Requirements — Based on interview conducted, the director was absent from the center for 2 months, which poses/posed a potential health, safety or personal risk to children in care.
Site visit notice posted at the facility — Based on recored review of 5 files, the licensee did not place the signed Acknowledgement of Receipt of Licensing Report (LIC9924) dated 4/8/26 in the childrens's file which poses/posed a potential health, safety or personal risk to children in care.
Carbon monoxide detectors required — Based on observation, the licensee did not comply with the section cited above in having a working carbon monoxide detectors which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in having an accisble rose bush with thorns in the outdoor area children use which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in having current Mandated Reporter Certifications for all staff which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — The Assistant Director disclosed that Staff 2 made a disclosure of Staff 1 mishandling a Child in a rough manner. It was also disclosed that Staff 1 was removed from their position temporarily and placed in the office and assigned office related work. Counseling was provided to Staff 1 regarding handling children in a rough manner. During the interview with Staff 3, it was disclosed that Staff 3 has observed Staff 1 on multiple occasions handling children in a rough manner. This poses a potential health, safety, and personal rights to children in care.
