Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above as LPM/LPA observed inadequate cushioning under play structures and fall zones, tricycle missing rubber handle exposing metal, other handle has a torn rubber handle, wooden swing set and wooden hose container has splinters on wooden surface which poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on rrecord review, the licensee did not comply with the section cited above as LPM/LPA observed not all children's files contained a singed copy of admission agreement with all the required services listed including but not limited to Title 22 Section 101200(b) and (c) which poses a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on oobservation, the licensee did not comply with the section cited above as during during facility inspection LPM/LPA one functioning window did not have a screen which posed a potential health, safety or personal rights risk to persons in care.
