Responsibility for Providing Care and Supervision — the observation during today's inspection. Based on observations, interviews, and record reviews, the facility did not comply with the section cited above A child was left behind in classroom during transitioning children to playground This poses a potential health and safety risk to children in care.
Child’s Personal Rights Section 101223 (a)(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of a punitive nature…, Staff spoke to child in raised voice as child was not listening to staff, which posed an immediate risk to the health, safety, or personal rights of children in care.
Buildings and Grounds — Based on record review, the licensee did not comply with the section cited above as large play structure (blue pole) in playground has a loose screw/bolt, small play structure's stair is in disrepair during today's inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
Disaster Drills shall be conducted at least every six months. — Based on record review, the licensee did not comply with the section cited above as last fire drill was documented as conducted on 2/19/25, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on record review and interview, a staff member violated a child's personal rights which poses a potential health and safety risk to children in care.
Reporting Requirements — Based on interview and record review, the Director did not report the incident or law enforcement presence at the facility which poses a potential health and safety risk to children in care.
