Staffing Ratio and Capacity — Based on observation, interview, and record review, LPA observed only Assistant #1 (A1) providing care to four infants, one preschool-age child, and two school-age children for a total census of seven children. Upon arrival, LPA inquired if Licensee was present. Per A1, Licensee was not present in the home. Per record review, A1 did not have a current Pediatric CPR/FA, which poses an immediate health, safety or personal rights risk to persons in care.

Staff immunization — influenza, pertussis and measles — Based on record review, Assistant #1 (A1) proof of immunizations (MMR, Tdap) not included in file at time of inspection which poses/posed a potential health, safety or personal rights risk to persons in care.

Family home — safe toys and play equipment — Based on record review and observation, the licensee did not comply with the section cited above in Child 1 was observed sitting in a Bumbo Baby Sitter Seat on the floor in the playroom. Per CPSC this item was recalled. This poses a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above when four faceplate covers were missing, which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in two of six child files reviewed did not contain documentation of the immunizations (child 1 and child 2). which poses/posed a potential health, safety or personal rights risk to persons in care.

Definitions — Based on interviews, care and supervision was provided to C1 in the licensee’s home for periods of more than 24 hours. This poses a potential Health, Safety, or Personal Rights risk to persons in care.

Immunizations — Based on interviews and record review C2 was enrolled on September 17, 2024, and did not have the required immunizations prior to enrollment. This poses a potential Health, Safety, or Personal Rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above when C1 (15 month old infant) was observed asleep in the play yard. C1 was laying on a loose object, a boppy pillow within the play yard which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above when C1 was asleep in a play yard with a pacifier in C1s mouth. The pacifier was attached to C1s clothing, which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in three out of six child files were missing immunization information, which poses/posed a potential health, safety or personal rights risk to persons in care.
