Staffing Ratio and Capacity — Based on observation and record review, the licensee did not comply with the section cited above as LPAs observed 10 daycare children present, five of whom were infants, which poses an immediate health, safety or personal rights risk to persons in care.
Personal Rights — Based on observation, the licensee did not comply with the section cited above as LPAs observed two daycare children in highchairs for approximately 10 minutes without food and LPAs observed one of the two daycare children fall asleep in the high chair for approximately 10 minutes which poses an immediate health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee and S1 were both missing current Mandated Reporter Child Care Provider certificates (AB1207). Licensee's AB1207 expired on 11/2025 and S1 did not have a AB1207 certificate at all which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that S1 was missing proof of the pertussis immunization and measles immunization which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on observation and record review, the licensee did not comply with the section cited above as LPAs observed an infant in a rocker located on top of a changing table. Although S1 was next to the infant, according to page 2 of the user manual found on Amazon.com, it states use rocker "only on the floor, never use on an elevated surface" which posed a potential health, safety or personal rights risk to persons in care.
Application for Initial License — Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that S1 did not have proof of Tuberculosis clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.
