Criminal background check — Based on observation, interview and record review, the licensee did not comply with the section cited above as in S12 and an armed guard do not obtain criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.

Health-Related Services — Based on observation, the licensee did not comply with the section cited above, LPAs observed two Epi- pens and Benadryl in childrens cubby located in the restroom in Pre nursery classroom, Room #2 which poses an immediate health, safety or personal rights risk to persons in care.

Carbon monoxide detectors required — Based on observation and interview review, the licensee did not comply with the section cited above as in the facility does not obtain a smoke and carbon monoxide detector at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on observation, interview, record review, the licensee did not comply with the section cited above as in facility has not completed a Disaster drills within the last 6 months, which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above a the bathroom in room 106 was not kept clean for children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on observation, interview, record review, the licensee did not comply with the section cited above as in Staff 3 did not obtain immunizations against influenza, pertussis, and measles in their file for LPA to review which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Records — Based on interview record review, the licensee did not comply with the section cited above in facility did not provide LPA 6 out of 11 files for review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Waivers and Exceptions for Program Flexibility — Based on observation interview record review, the licensee did not comply with the section cited above as the facility did not make available for review proof of their granted waivers, which poses/posed a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on record review, the licensee did not comply with the section cited above, Per director last drill was conducted 12 months ago which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above inLPAs reviewed staff files and did not observe staff #5, #6 and staff #7 to have Mandated Reporter training in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in child #3 and child #5 did not contain physicians report in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on record review, the licensee did not comply with the section cited above, LPAs conducted childrens file review, files for child #1 and child #2 did not contain in writing instructions from the childs authorized representatives for the administration of medication to the child. which poses/posed a potential health, safety or personal rights risk to persons in care.
