Outdoor Activity Space — At 10:36 AM, based on LPA observation of outside play area, Licensee did not meet the above requirement. LPA observed multiple spider webs on play equipment such as bicycles and crawl tube as well as spider webs on bottoms of fence which is accessible to children. LPA observed debris such as plastic bags and candy wrappers through floor of yard. These observations pose a potential health, safety, and personal rights risk to children in care.
Staff immunization requirements — Based on LPA record review, LPA did not observe immunization records in S1 and S3's file. This poses a potential health, safety, and personal rights risk to children in care.
Mandated reporter training — Based on LPA record review, LPA did not observe current proof of completion for AB1207 Mandated Reporter Training on all three staff files at time of inspection. This poses a potential health, safety, and personal rights risk to children in care.
Personnel Requirements — Based on LPA record review, LPA did not observe a health screening (LIC 503) in S2 an S3's files at time of inspection. This poses a potential health, safety, and personal rights risk to children in care.
Child's Medical Assessments — Based on LPA record review and interview, Licensee did not meet above requirement due to C3 not having Physician's Report on file. C3 has been enrolled in facility since 09/2022. This poses a potential health, safety, and personal rights risk to children in care.
Immunizations — Based on LPA record review and interview, LPA did not observe immunization records for C3 and C5 at time of inspection. This poses a potential health, safety, and personal rights risk to children in care.
