Responsibility for Providing Care and Supervision — LPA observed a child go in from the playground without staff knowledge. LPA walked the child over to another staff member inside the facility, and child stayed with staff indoors until another staff came in to help, which poses an immediate health, safety, or personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — Director did not comply with the section cited above as witness and interview statements revealed that on at least one occasion, a child from the infant license was taken to a preschool classroom due to staffing issue, which poses a potential Health, Safety, or Personal Rights risk to persons in care.
