Infant Safe Sleep — Based on observation and interview with licensee Olimpia, the licensee did not comply with the section cited above in 1 of 1 infant, which poses an immediate health, safety or personal rights risk to persons in care. Upon records review, the LPA observed no 15-minute checks. The licensee stated she does not do the 15 minute checks.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in Licensee and assistant did not have a mandated reporter training certificate in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above in the assistant did not have her immunization record in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on record review, the licensee did not comply with the section cited above in 3 out of 5 children's files were not complete which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in the licensee did not have the 15 minutes check documentation which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on records review and interview, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. The licensee stated she has been conducting and documenting the 15 minutes infant safe sleep checks and logs, but the documents are in co-licensee Alexis's vehicle. Alexis was not home at the time of the visit. Licensee Olimpia did not have proof of safe sleep logs for 3 infants present.
Mandated reporter training — Based on a records review, the licensee did not comply with the section cited above for three staff (two of whom were present; S1 and S3), which poses/posed a potential health, safety or personal rights risk to persons in care. The mandated reporters in file for the licensees S1 and S2 were dated 6/22/22 and 5/27/22 respectively. The licensee did not have proof of renewal of the mandated reporter in file for licensees or assistant.
Staff immunization — influenza, pertussis and measles — Based on records review, the licensee did not comply with the section cited above in two of three staff (S2 and S3; S3 was present), which poses/posed a potential health, safety or personal rights risk to persons in care. The licensee stated she did not have assistant S3's file available for inspection at the time of the visit. The licensee did not provide proof of immunization for Alexis or assistant S3.
Family home — home free of safety hazards — Based on records review, the licensee did not comply with the section cited above in 1 of 1 disaster drills, which poses/posed a potential health, safety or personal rights risk to persons in care. A records review shows that the last drill was documented on 5/17/2023, which is over 6 months prior to the date of the visit.
Staff immunization — influenza, pertussis and measles — Based on records review, the licensee did not comply with the section cited above in 1 of 1 volunteer, which poses/posed a potential health, safety or personal rights risk to persons in care. During records review the LPA observed the volunteer's immunizations were not in file.
