Reporting Requirements — Based on file review and interviews the Facility did not report an incident from early 2024 to the Dept in which a staff member pushed down on Child 1 with a sleeping mat This posed/poses a potential health, safety or personal rights risk to children in care.
Personal Rights — Based on staff and reporting party's interview, a staff did not prevent a daycare child to be harmed by another child. Staff was observed standing and watching the two children who were playing in the rug area. Staff did not intervene when the two children were fighting over a piece of toy.
Criminal Record Clearance — Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement is not met as staff Melissa Hernandez Fuentes (Hired 3/12/24) has been working at the facility and her fingerprint is not associated to the facility. This poses a potential health and safety risk to children in care.
