101223(a)(1) - Personal Rights: (a) The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. Based on the incident report submitted by the facility representative and LPA's interviews and record review where S1 grabbed C1 by the arms/wrist and walked with them, this is a potential risk to the health and safety of children in care if not corrected.
Written directives for drinking water lead testing — Based on record review, the licensee did not comply with the section cited above in the facility stated they exclusively use filter water but did not submit a written attestation to licensing office which poses/posed a potential health, safety or personal rights risk to persons in care.
Notice to parents that site visit reports are available — Based on record reviews, the licensee did not comply with the section cited above in 5 children's files were missing LIC 9224 dated 3/26/2024 and interview with Staff#5 disclosed that LIC 9224 dated 3/26/24 along with the report was not given to all the parents which poses/posed a potential health, safety or personal rights risk to persons in care.
