Personal Rights — Based on interviews and a review of a photograph, it was determined that facility staff failed to ensure a child’s personal rights were met. Staff were aware a child was wearing pants with a soiled crotch and no undergarments yet returned the child to care without addressing the child’s toileting and hygiene needs. This is poses a potential health, safety and personal risk to children in care.
Staff immunization requirements — Based on observation, interview and record review, the licensee did not comply with the section cited above in that immunization records for S1 and S2 were not available for review during time of inspection which poses a potential health, safety or personal rights risk to persons in care.
