Buildings and Grounds — LPA observed that the ABC rug continues to be used even though it still lifts/curls; however, now its all around the rug. According to the facility representative, they placed rug tape but it only lasted 3 days.

Buildings and Grounds — Based on observations in room K1: the ABC rug is lifting/curling, and the cement ramp with railings made of wood was removed; however, it will be replaced until Friday April 17 after operation hours. Room K2: the wood in the entrance of the room feels deep and soft when walking in/out. The edge of the floor has a gap and exposes the wood which is dark brown not the same color as the rest of the wood in the classroom. Light bulbs are either missing or not working. The light bulbs are also missing their covers which are currently exposed.

Fixtures, Furniture, Equipment and Supplies — Based on observation and interview, all equipment and materials for children were removed and thrown away when room K2 was being used by the Principals.

Staff immunization requirements — Based on observations, interview, and record review LPA observed that S2 is missing proof of MMR vaccine.

Reporting Requirements — Based on interviews, room K2 is no longer exclusive to the preschool program due to low enrollment. The facility did not report to the department that the Archdiocese administrators would be utilizing room K2. As of today, the administrators are still using room K2.

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above in the border around the carpet in room K1 is cracked, the ABC rug is lifting/curling, the white fence in the entrance of the facility has rust and deteriorating signs since its missing some metal parts and doesn't close properly, and the cement ramp with railings made of wood needs replacement due to not being sturdy and is falling apart which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 2 staff files reviewed is missing proof of MMR and Tdap vaccines which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 2 staff files reviewed is missing proof of the health screening and TB test which poses a potential health, safety or personal rights risk to persons in care.

Carbon monoxide detectors required — Based on observation, the licensee did not comply with the section cited above in facility does not have a carbon monoxide which poses/posed a potential health, safety or personal rights risk to persons in care.

Drinking water lead testing — Based on observation, the licensee did not comply with the section cited above in the facility has not completed the LEAD test which poses/posed a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on observation, the licensee did not comply with the section cited above in facility does not have a disaster drill which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in border of the wood floor is broken which poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in shade tarp has some tears on the corners which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on observation, the licensee did not comply with the section cited above in staff #1 and 2 have not completed the mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on observation, the licensee did not comply with the section cited above in Staff #1 and #2 do not have proof of influenza vaccine. Staff #2 does not have proof of MMR and Tdap which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on observation, the licensee did not comply with the section cited above in Staff #1 and #2 have completed the Pediatric CPR/FA; however, they don't have proof of a current CPR/FA which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on observation, the licensee did not comply with the section cited above in Child #1 and #2 are missing medical assessment which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on observation, the licensee did not comply with the section cited above in Child #1 and #2 are missing the emergency medical treatment form which poses/posed a potential health, safety or personal rights risk to persons in care.
