Teacher-Child Ratio.(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This requirement was not met: Based on interviews conducted and records reviewed, the facility was out of ratio on 6/30/25 and 7/1/25. This poses an immediate risk to the safety and health of the children in care.

Teacher Qualifications and Duties — Based on staff interviews and record review, the facility did not comply with the section cited above in S7, S8, and S9 have been a left alone with the children and do not have the qualifications which poses/posed a potential health, safety or personal rights risk to persons in care.
Limitations on Capacity and Ambulatory Status — Based on 11 staff interviews, it was determined that on August 8, 2025, there was a soccer clinic for the children in a building that is not licensed.
Reporting Requirements — Based on record reviews, the facility did not report the 2/24/25 & 2/28/25 incidents to the Department as required. This is a potential risk to the safety of the children in care.
