Family home — home free of safety hazards — Based on record review and interview, the licensee did not comply with the section cited above. Upon file review, it was confirmed that child #6 was missing Identification and Emergency Information Form and did not have Consent for Emergency Medical Treatment (LIC 627) form. This poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review and interview, the licensee did not comply with the section cited above. Child #6 did not have a nap log documenting every 15 minutes. This which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on observation, the licensee did not comply with the section cited above. LPA Ocegueda observed that the kitchen walls had multiple smudge stains, splatter stains and looked generally unclean. Other surfaces in the kitchen also appeared unclean (stove, and counters) with excess smudges and old food particles. There were multiple wall outlets that did not have safety covers in the kitchen and living room. There were two car seats in the living room today which were accessible to infants and children. This poses a potential risk to the health, safety or personal rights risk to persons in care.
