Family home — home free of safety hazards — Based on observations, the licensee did not comply with the section cited above. LPA observed cleaning supplies and chemicals in the hallway closet. The hallway closet door is broken, which made the items accessible to the children. LPA observed a polish and granite cleaner underneath the unlocked kitchen sink cabinet, which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on records review, the licensee did not comply with the section cited above. During today's inspection, Licensee was unable to provide LPA with proof of Mandated Reporter course completion for Staff #1 and Staff #2. This poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on observation and record review, the licensee did not comply with the section cited above. Licensee had missing documents for Child #1 (Consent to Emergency medical Care form LIC627), Child #3 (immunization records) and Child #4 missing required documents, which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation and record review, the licensee did not comply with the section cited above. Child #3 is missing Individual Infant Sleeping Plan LIC 9227, which poses a potential health, safety or personal rights risk to persons in care.
