The licensee shall report the following information the Department by telephone or fax within the Department's next business day and during normal working hours (8am to 5pm). Any change in household composition including adults moving in or out of the home and anyone living in the home who reaches his or her 18th birthday. — Based on record review tat additonal adults and children were currently living in the home. This poses a potential health and safety risk to children in care.
Personal Rights — Based on interviews conducted and Licensee's own admission that children are allowed to urginate outside (nature pee). This poses a potential health and safety risk to children in care.
Advertisements and License Number — Based on observation and interviews LPA Tuba observed the facilities website and obtained enrollment contract. This poses a potential health and safety risk to children in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in there was no proof or expired mandated reporting certificate trainings for both assistants working at the child care which poses/posed a potential health, safety or personal rights risk to persons in care.
