Criminal background check — Based on interview and record review, the licensee did not comply with the section cited above. Licensee's daughter Nekone Garay was present in the home for at least five days from 05/03/2024 through 09/21/2024 without fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.
Personal Rights — Based on observation, the licensee did not comply with the section cited above. LPA observed an infant sitting in a high chair not being fed which poses an immediate health, safety or personal rights risk to persons in care.

Alterations to Existing Building or Grounds — Based on observation, interview and record review, the licensee did not comply with the section cited above. LPA observed room additions including a day care room and bathroom in the home. Licensee failed to inform the Department of changes which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above. Child 2, 3, and 5 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above. Child 2 is missing LIC627 which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on interview and record review, the licensee did not comply with the section cited above. PA observed licensee is not completing Infant Safe Sleep log properly and only documenting the date and time they sleep which poses/posed a potential health, safety or personal rights risk to persons in care.
