Personal Rights — Five of eight staff interviewed indicate that on more than one occasion they have witnessed S7 mistreat infants. Two of eight staff interviewed also indicate that on more than one occasion they have witnessed S8 mistreat infants. S7 admitted to mistreatment of children.

Personal Rights — LPA observed 1 infant on their back, with a pacifier, a bib, a blanket covering the infant from their neck down and laying on top of a boppy pillow. It was evident that the infant was as sleep which posed a potential health, safety or personal rights risk to persons in care.
Criminal Record Clearance — LPA observed that staff 1 (S1) fingerprints were not transferred/associated to the facility.
Infant Care Personal Services — Based on interviews and records reviewed it was revelaed that the facility did not meet the needs of children.
Personal Rights — Based on interviews and records reviewed it was revealed that the facility denied breakfast to C1 on 09/05.
Food Service — Staff #2(S2) gave C1 low fat milk during breakfast time not know C1 is lactose intolerant. Medical records and Parental Request iindicated C1 is to be provided lactose free milk.
Reporting Requirements — Allegation of mistreatment of children was not reported to the department.
