Infant Safe Sleep — Based on record review and observation, Infant #1 was enrolled on 07/15/2025. The age of the infant upon enrollment was approxmately 9 months plus a couple weeks, LPA did not observe a sleep chart or LIC9227 in the infants empty file, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
