Family home — children supervised at all times — … Based on information obtained during the complaint investigation, the licensee failed to ensure C1 was supervised at all times, resulting in C1’s injury on 9/10/25 and posing an immediate health and safety risk to children in care.

Criminal Record Clearance — Based on observation, interview, and records reviewed, the licensee did not comply with the section cited above as evident by the presence of uncleared person "Nargis Yadullahi" in the Family Child Care Home which poses an immediate health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on LPA's observation the Jacuzzi is accessible to children in care. The kitchen sliding door and playroom window both make it accessible to the Jacuzzi. This poses an immediate risk to the health and safety of children in care.

Reporting Requirements — Licensee failed to report to the department in a timely manner of an incident that occurred on 9/11/25 involving child who sustained an injury requiring medical evaluation, and subsequent involvement of third parties.

Family home — in-ground pool drowning prevention requirements — Based on observation and interview, the licensee did not comply with the section above (pertaining to bodies of water), in that she did not have the required pool cover or alarm in place. This poses a potential health, safety or personal rights risk to children in care.

Infant Safe Sleep — Based on observations, the Licensee did not comply with the section cited above as evident by 4 cribs were covered with sheets as children were napping.

Infant Safe Sleep — Based on documents reviewed and interview with the licensee, sleep logs were not maintained for 6 infants which poses potential risk to health, safety and personal rights of children in care.

Personal Rights — Based on observations, 2 children remained in highchair for at least 30 minutes. which poses a potential risk to health, safety and personal rights of children in care.
