Infant Care Aide Qualifications and Duties — Based on record review and interviews, the licensee did not ensure infant care aide, (S1) was working under direct supervision of a fully qualified teacher on July 9, 2025. This poses an immediate risk to the health and safety of persons in care.
Staff-Infant Ratio — Based on record review and interviews, the licensee did not ensure there was a ratio of one teacher for every four infants on July 9, 2025, when one aide was providing direct supervision to five infants between 5-6:00pm. This poses an immediate risk to the health and safety of persons in care.
Personal Rights — Based on LPA interviews, the Licensee did not ensure C1 was accorded dignity when S1 patted infants roughly on the back, and engaged with infants without support which poses/posed an immediate health, safety, or personal rights risk to persons in care.

Indoor Activity Space for Infants — Based on observation the licensee did not comply with the section cited above in when the barrier was between 19 inches and 30 inches in height and not the required four feet, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on observation, interview, and record review, the licensee did not comply with the section cited above when a zero staff members were on site with current CPR/First Aid training, which poses/posed a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on interviews disaster drills were not being conducted at least every six months. This poses a potential Health, Safety, or Personal Rights risk to persons in care.
Immunizations — Based on LPA record review and interviews, at the time of C1 admission, to the child care center C1 was not immunized against the diseases required in Title 17, which poses/posed a potential health, safety, or personal rights risk to persons in care.
Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on LPA interviews, blankets were used as extra support when C1 and C2 were seated at the toddler table, therefore the toddler table was not the right size for C1 and C2, which poses/posed a potential health, safety, or personal rights risk to persons in care.
Infant Care Center Sleeping Equipment — Based on LPAs observation and interviews, the Licensee did not ensure there was nothing attached to a pacifier when C1's pacifier was attached to a blanket while C1 was sleeping in a crib. This poses a potential risk to the health and safety of persons in care.
Outdoor Activity Space — THIS REPORT IS AN AMENDED REPORT TO THE ONE GIVEN TO THE FACILITY ON 07/18/2024. Based on observation, the licensee did not comply with the section cited above in when the wooden tabletop of the table in the outdoor activity space had a rough surface, which poses/posed a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision for Infants — Based on record review, the licensee did not comply with the section cited above on July 1, 2024, July 2, 2024, July 15, 2024, July 16, 2024, July 17, 2024, the staff initials of the person who conducted each check was not documented, which poses/posed a potential health, safety or personal rights risk to persons in care.
