LPA arrived when the children were napping and the logs were not completed.
Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above in that the backyard has worn toys and miscellaneous items, such as stacked trash bags which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above as Child 2 had a balnket in the crib which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in 5 out of 5 children did not have PM 286 completed which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above in that an area of the backyard has several items including but not limited to a spray can, unrolled garden hose, items stacked haphazardly, which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, the licensee did not comply with the section cited above in that none of the immunization records have been transferred to the PM286 and two children did not have the immunizations in their records, which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in that form was not in the file for C6 which poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on record review, the licensee did not comply with the section cited above in 6 out of 6 records, which poses a potential health, safety or personal rights risk to persons in care.
