Staffing Ratio and Capacity — Based on interview, Licensee stated that on 1/27/26, there were 10 children present at the facility. S1 stated that she was absent from the facility between approximately 11:10 AM and 11:20-11:30 AM which poses a potential risk to the health, safety, or personal rights of persons in care.
Child's Records — Based on record review and interview, facility roster of children in care did not contain entries for C1, C2, C4, and C6 which poses a potential risk to the health, safety, or personal rights of persons in care.
The family day care home shall record each pupil's immunization on the California School Immunization Record, PM 286 (6/95) This regulation was not met as evidenced by Based on interview, Licensee stated that she did not possess child files for C2 and C6 except for the immunization record but that it was not recorded onto the CDPH286 form which poses a potential health, safety or personal rights risk to persons in care.
