Personnel Requirements — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that staff #1 and staff #2 had completed CPR/First Aid training. However, they could not provide evidence that the training was "Pediatric" training.

Licensing Program Analyst (LPA) observed that the carpet in the classroom was highly stained with dark marks throughout.
Drinking water lead testing — Based on interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Site Supervisor informed Licensing Program Analyst (LPA) that the center's water had not been tested for lead. However, the school's administration had recently reached out to a company in order to begin the testing.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that all three staff members had completed an in house Mandated Reporter Training, which was not on the approved list for training.
