Family home — children supervised at all times — Based on observations, interviews and records obtained, the licensee did not comply with the section cited above as licensee was out of town while adult #1 was caring for a day-care child, which poses a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Based on LPA's observations, child # 2 and child #3 were in an “off limits” room, which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on LPA's observations, child # 1 was strapped in an infant car seat for approximately 5 minutes with a blanket and bottle, which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — During records review LPA Herrera did not observe immunization records for child #5 and incomplete records for child #3, which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — During file review LPA Herrera did not observe an "Individual Sleeping Plan" (LIC 9227) on file for child #5, which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — During records review LPA Herrera did not observe a 15 sleep minute log for child #5, which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that, LPA observed that Licensee did not update their Mandated Reporter Certificate which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee last completed their Mandated Reporter on 6/17/2021.
