Responsibility for Providing Care and Supervision — Based on observation and interview the licensee did not comply with the section cited above as seven children (infants and preschool children) were left in the care and supervision of a staff without verifiable qualifications in a room that was unsecured, including a broken baby gate and access to doors leading outside, which poses an immediate health, safety or personal rights risk to persons in care.

TrustLine provider — registry record not kept current — Based on observation, interview, and record review, the facility employeed an individual with a lifetime exclusion from working at any licensed child care facility that was effective as of October 2022.

Criminal background check — Based on record review and interview, the licensee did not comply with the section cited above. One person who was present did not have valid fingerprint clearance at the time of the inspection.

Criminal background check — Based on record review and interview, the licensee did not comply with the section cited above. Three persons (including 2 staff members) present the facility did not have a criminal record clearance and are not associated to the facility. This poses an immediate health, safety or personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — Based on LPA observation, the licensee did not comply with the section cited above as there was co-mingling of toddlers and preschool children in one classroom (the main room, at entrance), which poses an immediate health, safety or personal rights risk to persons in care.

Preschool Program with Toddler Component — Based on observation, interview and record review, the licensee did not comply with the section cited above as toddler C1 was observed co-mingled with preschool children in the preschool room which poses/posed a potential health, safety or personal rights risk to persons in care.

Current roster of children — Based on record review and interview, the facility did not maintain a currrent roster, as child C1 ws not accounted for, which poses a potential health, safety or personal rights risk to persons in care

Food Service — Based on observation, interview, and record review, the licensee did not comply with the section cited above as the meal served did not meet the nutritional needs of the children as whole milk was served and no vegetable was present, which poses a potential health, safety or personal rights risk to persons in care.

Operating without a license — (1) Whenever milk is served, serve only lowfat (1 percent) milk or nonfat milk to children two years of age or older. Based on observation and interview, the facility supplied whole milk at parent request instead of 1% milk, which poses a potential health, safety or personal rights risk to persons in care

Notice to parents that site visit reports are available — Based upon interviews and records review, the facility delayed required dissemination and collection of LIC9224 forms, which provide proof that licensing reports were received. This is a potential health, safety and personal rights risk to children in care.

Reporting Requirements — Based on observation and interview, the licensee did not comply with the section cited above as there has been a structural incident involving the ceiling in the toddler room (honeybees) rendering the room unusable, which was not reported, which poses a potential health, safety or personal rights risk to persons in care.

Child Care Center Director Qualifications and Duties — Based on observation, interview, record review, the licensee did not comply with the section cited above as there was only one staff member whose qualification could not be verified was present and was not able to operate or act in place of the director, including knowledge of facility operation, which poses/posed a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on observation, interview, and record review, the licensee did not comply with the section cited above as the emergency disaster plan provided to the department did not have assigned duties for any current staff members and a relocation site that is no longer open which poses a potential health, safety or personal rights risk to persons in care.

Admission Policies — Based on observation and record review, the licensee did not comply with the section cited above as the department did not see PUB 393 Child Care Center Notification of Parents' Rights Poster in a prominent or publicly accessible place which poses/posed a potential health, safety or personal rights risk to persons in care.

Drinking Water — Based on observation and interview, the licensee did not comply with the section cited above as water was not available in the room, in which care is being provided, and the collection of said water would impact the supervision of children, rendering the water inaccessible which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on observation, interview, and the lack of record review, the licensee did not comply with the section cited above as the facility did not provide the department with children's records for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on observation and interview, the licensee did not comply with the section cited above as 5 out 8 required first aid items were not present which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, interview, and record review, the licensee did not comply with the section cited above as there is a structural deficiency (a fallen and broken ceiling panel) in the toddler room (honeybees), a broken child gate allowing children to exit the classroom, and plastic bags (small and large), and with dirty diapers accessible to children which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on observation and interview, the licensee did not comply with the section cited above as the available teacher could not meet the needs of the children including, proper diapering, water availablity, or meal preperation, which poses/posed a potential health, safety or personal rights risk to persons in care.

Preschool Program with Toddler Component — Based on observation, interview and record review, the licensee did not comply with the section cited above as toddlers were observed co-mingled with the preschool children which poses/posed a potential health, safety or personal rights risk to persons in care.

Food Service — Based on observation, interview, and record review, the licensee did not comply with the section cited above as the food present did not meet the minimal nutritional needs of the children or to supply the food listed on posted menu's and with no alternative meals ready or available which poses/posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation and interview, the licensee did not comply with the section cited above as there was no sanitary equipment or conveniences available to children in care, such as toilet paper or paper towels for potty trained children, which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Personal Services — Based on observation and interview, the licensee did not comply with the section cited above as there was no appropriate receptical in place for soiled diapers and they were observed disposed of in an open air plastic trashbag accessible to children which poses a potential health, safety or personal rights risk to persons in care.

Personnel Records — Based on observation, interview, and the inability to conduct a record review, the licensee did not comply with the section cited above as the facility did not provide any personnel records to the department for review which poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on interviews the facility did not ensure supervision of child C1, as child was able to push open an exit door leading to a parking lot, which posed a potential Health, Safety or Personal Rights risks to persons in care.

Personnel Requirements — Based on observation and record review, LPA observed that one staff member was left supervising seven children without there being a staff member with first aid and CPR.

Teacher Qualifications and Duties — Based on observation and record review, LPA observed that one staff member was left supervising seven children without a fully qualified teacher to supervise both aides and children.

Fixtures, Furniture, Equipment and Supplies — Based on observation, LPA observed that the children were left in a pitch-black room that did not allow for adequate supervision of the children to ensure their safety while napping.

Personnel Requirements — Based on records review, staff, S1 was missing a completed LIC 503 Health Screening or equivelent in their records, which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on records review staff S1 did not have full immunizations proof in their file which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. Staff S1 was missing Mandated Reporter Training on file. This poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — LPA observed construction materials labeled “taping & topping joint compound” in the patio used for toddler play, which poses a potential health, safety or personal rights risk to persons in care.

Teacher Qualifications and Duties — Based on LPA observation and interview, staff Sukhri Abdi was observed supervising 6 children alone, when the LPA arrived. Ms. Abdi identified herself as an aide and no qualified teacher was present at the facility, which poses a potential health, safety or personal rights risk to children in care.

Personnel Records — Based on interview, the licensee stated that she did not have access to the personnel records for LPA review at the time of the inspection, which poses a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on records review, 14 of 14 children enrolled in the facility did not have any required children's documents/records which poses a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on interview, the licensee did not comply with the section cited above, as they did not furnish to the Department a report of a Director change at the facility, which poses a potential health, safety or personal rights risk to persons in care.

Food Service — Based on LPA observation, the licensee did not comply with the section cited above, as in the kitchen, several bottles of cleaning supplies were sharing a storage crate with canned goods and cooking oils, which poses a potential health, safety or personal rights risk to persons in care.
