Personal Rights — Based on interview and record review, the licensee did not comply with the section cited above as it was reported by the Licensee that a day-care child received a minor scratch from the Licensee's dog which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C2 was missing updated immunizations and C4 was missing proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.
