Infant Safe Sleep — Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 infants in care were missing sleep log which poses a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above in 1/3 far back portion of the back yard containing trampoline in outdoor play area with no off-limits barricade or fencing which poses/posed a potential health, safety or personal rights risk to persons in care.
