Operation of a Family Child Care Home — Based on record review, licensee did not comply with the section cited above in that the licensee did not maintain copies of LIC 282 Affidavit Regarding Liability Insurance for child #1, #2, #3 and #4 which posed a potential Health, Safety or Personal Rights risk to persons in care.

1796.45 TB Testing (a) Affiliated home care aides hired on or after January 1, 2016, shall submit to an examination 90 days prior to employment or within seven days after employment to determine that the individual is free of active tuberculosis disease. — Based on interview and record review, the licensee did not comply with the section cited above in that assistant #1 do not have TB skin test on file which posed a potential health, safety or personal rights risk to persons in care.
