Personal Rights — Based on observation and interview, the licensee did not comply with the section cited above. LPA observed one infant in a high chair without being fed. LPA addressed the infant in the high chair and assistant stated she had sat the child in the chair to eat her lunch. Assistant then sat another infant in a different high chair without feeding the child. Infants sat in the high chair for approximately 35 minutes. LPA observed preschool children sleeping in play yards that were to small for their sizes which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above. Assistant is missing pertussis immunization which poses/posed a potential health, safety or personal rights risk to persons in care.
