(1) No child(ren) shall be left without the supervision of a teacher at any time This requirement was not met as observed by: On 1/21/2026, staff failed to provide adequate supervision when a 3-year-old child was left unattended in the preschool play yard for approximately 1–2 minutes during a transition from outdoor play to the classroom.
