Personal Rights — Based on interview and record review the converted garage used by children was not kept at a comfortable temperature on 11/05/25 while used for napping. Additionally, Licensee had knowledge that the converted garage was converted without the permits prior to licensure and had prior experience with code enforcement, with knowledge that changes to the garage required permits, both which pose potential risk to the health and safety and personal rights of children in care.
Family home — home free of safety hazards — Based on observation at 1:20pm LPA oberved an infant sitting in a baby walker outside in the play yard, which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in that at 3:30pm, Licensee stated that her Mandated Reporter Training expired 06/2023, and she has not renewed it, which poses a potential health, safety or personal rights risk to persons in care.
Family home — preventive health training, including pediatric CPR and first aid — Based on interview at 4:06pm Licensee stated that on 10/08/22 she completed the online portion of the Pediatric First Aid/CPR course, but did not complete the in person portion and does not have a current certificate, which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on interview and record review, the licensee did not comply with the section cited above. Four out of the 4 infants present did not have sleep logs on file. Licensee stated that she does not currently have sleep logs which poses a potential health, safety or personal rights risk to persons in care.
