Site visit notice posted at the facility — 8/13/26 at 9:25AM LPA observed Notice of Site Visit(NOS)was not posted at entrance of facility. Director stated NOS is removed every Wed & Fri. Director forgot to post NOS on 8/13/26.This posed a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — Based on interviews and record review, three (3) staff stated that they have visually observed children biting due to a lack of supervision in room one which poses a potential risk to the health and safety or personal rights to children in care.
