Miraculous Milestones

Responsibility for Providing Care and Supervision — Based on interviews and record review, Child 1 (C1) was left unattended in the bathroom by Staff 12 (S12). C1 was not provided care and supervision during that time. This is an immediate risk to the health and safety, and personal rights risk to children in care.

Teacher-Child Ratio — Based on interviews and record review, S2 stated they were over 12 children by themselves.

Limitations on Capacity and Ambulatory Status — LPA was notified that on 6/11/24, Staff witnessed a child in the preschool room that was under the age of 2

Responsibility for Providing Care and Supervision — Based on interviews and record review, D3 found Child 1 (C1) unattended in the T-K classroom. C1 did not have care and supervision in that period of time. This is an immediate risk to the health, safety, and personal rights risk to chidlren in care.

Teacher-Child Ratio — Based on interviews and record review (sign in/out sheets), on 5/9/24 at 11:17am, at the time of the incident D6 and D7 had 19 preschool children. This is out of ratio and an immediate risk to the health, safety, and personal rights risk to the children in care.

Site visit notice posted at the facility — 8/13/26 at 9:25AM LPA observed Notice of Site Visit(NOS)was not posted at entrance of facility. Director stated NOS is removed every Wed & Fri. Director forgot to post NOS on 8/13/26.This posed a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on interviews and record review, three (3) staff stated that they have visually observed children biting due to a lack of supervision in room one which poses a potential risk to the health and safety or personal rights to children in care.

Reporting Requirements — Based on interviews and record review, Child 1 (C1) was left unattended in the bathroom by Staff 12 (S12). Facility did not report incident to the Department in a timely manner. This is a potential risk to the health and safety to children in care.

Teacher Aide Qualifications and Duties — Based on interviews and record review, S7 does not have units and is not a fully qualified aide or teacher which is a potential health, safety, and personal rights risk to the children in care.

Notice to parents that site visit reports are available — LPA observed that the notice of site visit from 5/13/24 Unusual incident report was not posted in the T-K classroom which poses a potential risk to the health, safety, personal rights risk to children in care.
