Buildings and Grounds — During the outdoor inspection, LPAs observation the licensee did not comply with the section cited above. LPAs observed the sensory table, two children kitchen aides, and the sand box cover collected rained water, wet sands, and dried leave that is unsafe and unsanitized to the children in care. This deficiency was previously addressed; however, LPAs observed that it is still poses a potential health, safety, or personal rights risk to children in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above. The following deficiencies: the exterior wall panels of the shed were tearing apart in the bottom of the shed; the children wooded outdoor play kitchen have 4 black widow spiders, bird drops and dried debris on the sidewalk cement, and multiple trash empty chip bags on the side of the fence. This poses a potential health, safety, or personal rights risk to children in care.
Reporting Requirements — Based record review, the licensee did not comply with the section cited above. Staff #5 (S5) is the site supervisor since 02/02/2023, but licensee failed to report the change of Director to the Department within 10 days.
