Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. The Licensee's Mandated Reporter Training expired in April of 2026.
Infant Safe Sleep — Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. The Licensee has not been documenting 15 minute sleep checks for children 24 months and under as required by the Department.
