Health-Related Services — Based on observation and record review, the licensee did not comply with the section cited above in that children # 1-3 have expired medication on cite which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation, interview and record review, the licensee did not comply with the section cited above in that Staff # 4, # 5 and # 8 do not have current AB1207 mandated reporter training certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Health-Related Services — Based on observation, the licensee did not comply with the section cited above in 2 out of 7 children's medication were not in their original box, which poses/posed a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in that there was three Clorox Bleach spray bottles and one Borax detergent box found under the sink in Room 5 that were accessible to children. Which poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on interviews conducted it was determined that the personal rights of Child # 3 were violated by Staff #4 This poses a potential health and safety risk to children in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that Staff # 5 does not have a current Mandated Reporter certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
