Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in that none of the facility staff had current mandated reporter certificates which posed a potential health, safety or personal rights risk to persons in care.
Modifications to Infant Needs and Services Plan — Based on observation, interview and record review, the licensee did not comply with the section cited above in that many infant children had expired Needs and Service plans which poses/posed a potential health, safety or personal rights risk to persons in care.
