Criminal Record Clearance All individuals subject to a criminal record review shall obtain a criminal record clearance or exemption and be associated to the facility prior to working, residing, or volunteering in a licensed child care center. The requirement was not met as evidence by: The licensee did not ensure that Staff #2 (S2), employed at the facility since 9/2024, is associated to the facility in the Guardian system. Based on file review, S2 was not associated from 3/2024 to 3/2026 to the facility. This posed a potential risk to the health and safety of persons in care.
Buildings and Grounds — Based on observation of the Outdoor play area, there was a spider under a preschool slide. This is a potential risk to the health and safety of children in care. This posed a potential risk to the health and safety of persons in care.
Buildings and Grounds — Based on observation of the Outdoor play area, there was a broken/rusty lunch table, broken playhouse, and a metal wire snap mouse trap in the director's office that is accessible to children. This posed a potential risk to the health and safety of persons in care.
Reporting Requirements — Based on interview with the Director, the injury to C1 was not reported to the Department as required. This is a potential risk to the health and safety of children in care.
Limitations on Capacity and Ambulatory Status — Based on interview the Director and staff, and review of documentation, the facility is commingling children from the toddler option program with the preschool program, which is beyond the limitations of the license and a potential risk to the health and safety of children in care.
Teacher-Child Ratio — Based on LPA's observation, Room #1 was out of ratio at 9:30AM, there was one teacher supervising 17 children when the other teacher was observed in the adjoining room on the telephone and not actively supervising children.
Reporting Requirements — Ms. Crisman failed to notify the department when she designated the new designated site director in 2023.
Food Service — Based on LPA's observation, on 12/7/23 the facility had snack menus dated 5/2021 & 5/2022 posted and on 1/26/24 the facility had menus dated 12/2023 posted. Interviews with staff and parents also indicate that current menus are not posted. This poses a potential health, safety or personal rights risk to the children in care.
