The requirement was not met as evidence by: The licensee did not ensure that Staff #2 (S2), employed at the facility since 9/2024, is associated to the facility in the Guardian system. Based on file review, S2 was not associated from 3/2024 to 3/2026 to the facility. This posed a potential risk to the health and safety of persons in care.
