No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This was not met as evidenced by:Based on the interviews with staff, parents, and children,and the physical plant inspections, LPA has determined that children were not supervised correctly on the date 8/3/21 at approximately 1pm on the outdoor play yard, which led to an innappropriate interaction between 3 children. This is an immediate threat to the children's health and safety.
A teacher shall not be required to perform housekeeping or maintenance duties that prevent him/her from performing duties related to providing care and supervision to children. This was not as evidenced by:Based on the interviews with staff, parents, and children,and the physical plant inspection, LPA has determined that children were not supervised correctly on the date 8/3/21 at approximately 1pm on the outdoor play yard, which led to an innappropriate interaction between 3 children. It was further determined that a staff #4 was performing house keeping duties at that time. This is a potential threat to the children's health and safety.
A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children. This was not met as evidenced by: Based on LPAs observation of the classroom and census taken, it was determined that one teacher was supervising 15 school age children. This was observed to occur during the inspection by LPA on the morning of 10/5. This is an immediate threat to the health and safety of the children in care.
