Buildings and Grounds — Based on observation and interviews, the licensee did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to persons in care. The classroom was filled with a pungent amonia like smell the LPA identified as cat urine. There were two bags of soiled cat litter under the sink in the children's restroom and a carboard box with cat litter in a corner. The director stated the cat litter is taken out daily. LPA observed at 11:46 am. one bag of soiled cat litter and the litter box were still in the restroom. Dried cat vomit was also observed in the classroom.

Outdoor Activity Space — Based on observation and interview, the licensee did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to persons in care. The LPA observed cat feces in various areas of the outdoor activities area, including the sand under the swings, the sand under the monkey bars, and the grassy areas. The director stated the playground was cleaned when the feces were observed. The LPA observed dried feces and fresh feces in 13 spots in the outdoor activities area.

Personal Rights — Based on observation and interivew, the licensee did not comply with the section cited above, which poses an immediate health, safety or personal rights risk to persons in care. The LPA observed child #1 (C1) using the outdoor activity area before the observed cat feces were removed the area. C1 was dragging a water hose to water plants in the outdoor area. C1 stated they play in the outdoor area and at times pick loquat from the tree in the outdoor activity area.

There is a pungent amonia-like smell in the classroom. The LPA identified it as dog/cat urine. A black cat and a chihuahua were observed roaming in the classrooms. A grey cat is also allowed in the classroom per licensee. LPA observed pet beds nad a cage in the classroom.

Personal Rights — Based on observation and interviews, the licensee did not comply with the section cited above in one of one outdoor activity areas, which poses an immediate health, safety or personal rights risk to persons in care. The children were observed playing around animal (cat like) feces in the outdoor area. The licensee stated they usually have it cleaned but not today. There were feces in the swings and monkey bar areas. One child was seating on the floor were feces were observed.

Carbon monoxide detectors required — Based on observation and interviews, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA observed the carbon monoxide detertor did not have batteries. Mrs. Perera stated the detector used to have batteries.

Mandated reporter training — Based on observation and records review, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. Staff 1 and Staff 2 did not have a valid mandated reporter in file.

Personnel Requirements — Based on observation the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA observed the CPR for Staff 1 and Staff 2 expired in 2023. S2 stated they were looking for a group to join to renew the CPR.

Limitations on Capacity and Ambulatory Status — Based on the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA observed elementary school children using the restroom. Staff 2 said the elementary children have a separate restroom that is not in use and they instead use the preschool restroom. There is no waiver to share the restroom.

Carbon monoxide detectors required — Based on observation and interviews, the licensee did not comply with the section cited above in one of one carbon monoxide detectors, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA observed the CO2 detector didn't have bateries. S2 said they didn't have batteries for the detector.

Drinking water lead testing — Based on observation and interview, the licensee did not comply with the section cited above in one of one drinking fountain in the classroom, which poses/posed a potential health, safety or personal rights risk to persons in care. S2 said the technician never went to test the water for lead.

Application for License — Based on observation and interviews, the licensee did not comply with the section cited above in one of one posting requirements, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA observed the telephone for the health deparment or the child passanger restrain system were not posted as required.

Indoor Activity Space — Based on observation, the licensee did not comply with the section cited above in one of one classrooms, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA observed dirt and other substance accumulated on walls and floor. The substance, which appeared to be dried liquid, left dripping marks on the walls.

Storage Space — Based on observation, the licensee did not comply with the section cited above in three of three children present, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA observed the children's backpacks were on the floor. The backpacks contained the water bottles and according to the licensee also the children's lunch boxes.

Buildings and Grounds — Based on observation the licensee did not comply with the section cited above in one of one cleaning compound, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA observed a green cylinder of "Comet" under the sink, accessible to children in care.

Activities/Napping — Based on interviews, the licensee did not comply with the section cited above in three of three children present, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA inquired about nap/relaxation time. The director stated the children don't nap because their parents don't want them to take a nap. A child under 5 was present in care at the time of inspection. The facility doesn't have a scheduled nap time.
