Sign in and Sign out — Based on LPA's review of facility records (sign in and sign out) and staff statements, some parents do not sign their children in and or out.
Buildings and Grounds — Based on observation, the facility front entry gate is broken and does not latch properly. LPA arrived at the facility and observed the front gate wide open.
