Notice to parents that site visit reports are available — This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as C1, C4, C5, and C6 are missing proof of completed LIC9224's that have been issued within 12 months, which poses an immediate health, safety or personal rights risk to persons in care.

Infant Care Teacher Qualifications and Duties — Based on observation and record review, the licensee did not comply with the section cited above as LPAs observed S1, who is not a qualified teacher, left alone with infants who were awake which poses an immediate health, safety, or personal rights risk to children in care.

Responsibility for Providing Care and Supervision for Infants — information obtained showing two older infants next to a diaper changing table and other napping infants, left without supervision. This poses an immediate health, safety, or personal rights risk to children in care.

Staff-Infant Ratio — information obtained that infants are being left under the care of unqualified staff or aides on more than one occasion and facility being understaffed. This poses an immediate health, safety, or personal rights risk to children in care. There is a $1000.00 civil penalty being assessed due to this being a repeat citation.

Infant Care Personal Services — the information obtained that an infant/child was left unattended on a diaper changing table and was observed standing and moving around, while staff retrieved a diaper. This poses an immediate health, safety, or personal rights risk to children in care.

Infant Care Teacher Qualifications and Duties — Information was obtained that aides are being left alone with children and/or utilized for purposes of meeting ratios. This poses an immediate health, safety, or personal rights risk to children in care.

Infant Care Personal Services — Information obtained and observation made that infants are being diapered while standing in the restroom. This poses an immediate health, safety, or personal rights risk to children in care.

Infant Care Personal Services — An observation was made of a container that is utilized to dispose of soiled diapers which is not airtight, and odor is coming out of. This poses an immediate health, safety, or personal rights risk to children in care.

Infant Care Activities — Based on observation, Licensee did not comply with the section cited as two infants were observed asleep in swings. This poses an immediate health, safety, or personal rights risk to children in care.

Infant Care Center Sleeping Equipment — Based on observation, the Licensee did comply with the section cited as one infant was observed asleep in a crib with a pacifer with a clip holder attached. This poses an immediate health, safety, or personal rights risk to children in care.

Infant Care Teacher Qualifications and Duties — LPA observed an assistant, which does not have the required educational qualifications, provind care and supervision to 5 infants. This poses an immediate health, safety, or personal rights risk to children in care.

Infant Care Activities — LPA's observation of Infant sleeping on a swing. This poses an immediate health, safety, or personal rights risk to children in care.

Staff-Infant Ratio — the observation made, during today's visit of an assistant providing care for 5 infants. This poses an immediate health, safety, or personal rights risk to children in care. Please note that this is a repeat violation/citation and therefore a civil penalty ($250.00) is being issued.

Personnel Requirements — record review and observation that staff was present without a Criminal Record Clearance. This poses an immediate health, safety, or personal rights risk to children in care. Please note that a civil penalty ($400.00) is being issued.

Responsibility for Providing Care and Supervision — the information obtained that an infant/child was choking on and swallowed a staple. Neither staff present saw the incident develop and/or out how it occurred. Infant/child was transported to hospital for medical attention.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above, S3 Mandated Reporter Child Care Providers Training (AB1207) date of completion is 07/24/2022 and a current certificate is not available for review, which poses a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on observation and record review, the licensee did not comply with the section cited above, LPA observed two Epipens located in a zipped carrying case without a pharmacy label identifying C6 as the individual prescribed the Epipens, the date prescribed, expiration date, or prescribing physician, which poses a potential health, risk to persons in care.

Modifications to Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above, the Needs and Services Plans for C2 (dated 02/06/24), C3 (dated 11/10/23) and C5 (dated 03/12/24) are not current (dated and completed within the last 3 months), which poses a potential safety risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above, immunization records for C2, C4, and C5 were not available for review, which poses a potential health risk to persons in care.

Infant Care Food Service — Based on interview and observation, the licensee did not comply with the section cited above. At times, staff will place an infant on a highchair and hold the bottle for feeding. -------------------------------------------------->

Admission Agreements — In reviewing the facility’s potty-training procedure which reads, “we require that the child be at least 2 years of age”, yet, per facility Director, they are potty training infants under 2 years of age. This poses a potential health, safety, or personal rights risk to children in care.

Infant Care Personal Services — In reviewing children files. Child#1 was being potty trained, per facility Director but, no plan/documentation or signed authorization was available for review. This poses a potential health, safety, or personal rights risk to children in care.
