Activities/Napping — Based on interview and record review, the facility did not comply with the section cited above given that there were 14 preschool children (13 napping/resting and 1 not napping/resting) in Room 7 with 1 staff for approximately 10 minutes.

Limitations on Capacity and Ambulatory Status — Based on observations, interviews conducted, and record review it was determined that during opening and closing times, the children in Room 2 and Room 3 combine which poses a potential risk to children in care.
Teacher-Child Ratio — statement of S4, S8, and S15 confirming an aid was supervising few children in one room for 10 minutes on one occasion, S6, and S7 confirmed S6 left a preschool room with 16-17 children under S7 supervision for a few minutes to assist a parent one time. S8, S11, and S15 reported hearing staff requesting assistance over walkie-talkies for being out of ratio. This is a potential risk to the health and safety of children in care.
Personal Rights — interviewing staff that S1 admitted raising their voice in frustration when addressing a staff's mistake in the presence of younger children. S2, S3, and S4 witnessed or heard S1 raising their voice at staff, including one instance during parent drop-off and another while younger children were napping. S5 and S8 confirmed hearing S1 yell at a staff member in the presence of younger children and parents. This is a potential risk to the health and safety of children in care.
