Outdoor Activity Space — Based on LPA's observation and director confirmation, the licensee did not comply with the section cited above and red table next to the sand box and pink old kitchen and storage in the corner of playground by the fences, the paint was chipped and not safe for children which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on five staff files reviews the licensee did not comply with the section cited above and staff#2 and staff#3 did not have proof of immunization against pertussis and staff#3 and staff #4 did not have proof of immunization against measles on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on five staff files reviews, the licensee did not comply with the section cited above in 1 out of 5 reviewed staff files, staff#4 did not have health screening form LIC 503 on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties — Based on 5 staff files review, the licensee did not comply with the section cited above in staff#1, who is a teacher did not have a copy of transcript on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on sign in/out records on procare, the licensee did not comply with the section cited above and during this inspection 33 children were present and 32 children were signed in which poses/posed a potential health, safety or personal rights risk to persons in care.
Advertisements and License Number — During 03/16/26 inspection, director, Shonna Clark checked the facility website and confirmed facility number is not posted on the facility website. This poses a potential health, safety and personal right risk to the children in care.
Staff immunization requirements — Based on observations , interviews and records review, 1 out of 10 did not have immunizaion records , which poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on the information obtained child #1 had allergic reaction on the facilty grounds. Staff called 911 and Epi pen was provided .
