Criminal Record Clearance — Based on observation, interview, and record review, the licensee did not comply with the section cited above in that, A1 did not have criminal record clearances prior to employment, which poses an immediate health, safety or personal rights risk to persons in care.

Evidence of a current tuberculosis clearance, not more than one year prior to or seven days after initial presence in the home, for any adult in the home during the time that children are under care. This requirement was not met. Based on the fact that Staff#4 did not have a TB test while residing in the home with children in care, which poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on interviews and record review, the licensee did not comply with the section cited above in that, six (6) children did not have a copy of their immunization record on file, which poses a potential health, safety or personal rights risk to persons in care.

Family home — admission procedures and parent rights — Based on interviews and record review, the licensee did not comply with the section cited above in that, a guardian was not permitted to enter the home or yard during operating hours, which poses a potential health, safety or personal rights risk to persons in care.

Licensees shall reveal each facility license number in all advertisements, publications, or announcements made with the intent to attract clients. This standard was net met as evidenced by the fact that the Licensee admitted to not including the facility number in her two advertisements.

Criminal Record Clearance — Based on observation, interview, and record review, the licensee did not comply with the section cited above in that, licensee did not submit transfer request for A2 and A3, so they were not transferred to facility, which poses an potential health, safety or personal rights risk to persons in care.
