101229 Responsibility for Providing Care and Supervision. (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This regulation was not met as evidenced by: LPA observed Child 1 climbing a book case and eating the tip off a marker without teacher's noticing. It was revealed that Child 1 was eating sand from the sandbox when under staff supervision. This poses a potential health, safety and personal rights risk to children in care.
101223 Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights:...(2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This regulation was not met as evidenced by: Child 1 was receiving diaper rashes while in care as well as wearing incorrect diapers. Parents share same concern of their child receiving diaper rashes and diapers that do not belong to the child. This poses a potential health, safety and personal rights risk to children in care.
Limitations on Capacity 101161(a) A license shall not operate a childcare center beyond the conditions and limitations specified on the license, including the capacit limitation. This was not met as evidenced by: Through interviews and document gathering, LPA learned school age children and preschool children comingle in the morning and evening. This poses a potential health and safety risk to children in care.
