Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above in 1 play yard another mattress was added to prevent child from hitting their head on wall causing sheets to be not fitted which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in 1 play yard has a sheet attached to the side and a blanket hanging over the side which poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on record review, 1 out of 1 infant under age one was missing the LIC 9227, therefore the licensee did not comply with the section cited above, which poses a potential health, safety or personal rights risk to persons in care.
