Personal Rights — Based on interviews conducted, children's personal rights were violated. This is an immediate risk to the health, safety and personal rights of children in care.
Health-Related Services — Based on observation and record review, the licensee did not comply with the section cited above. There were children's medications stored outside of the original packaging and prescription was missing. This poses an immediate health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation here were items found in the restroom and shared space of the center. These items included: "Microban sanitizing spray", "Cleaning Vinegar", "Kamar Varnish", "Shave Gel", "Disinfecting Laundry Bleach' and "Caldesene Medicated Protecting Powder". These items were not observed behind a lock, as stated in Title 22 Regulations. This poses an immediate health, safety or personal rights risk to persons in care.

(e) All personnel shall be given on-the-job training in the areas listed below,/ shall have related experience that demonstrates knowledge of & skill in those areas. Such training or experience shall be appropriate to the job assigned and shall be evidenced by safe and effective job performance. A teacher-child ratio of one teacher supervising 24 napping children is permitted provided that the remaining teachers necessary to meet the overall ratio specified in Section 101216.3(a) are immediately available at the center. During the facility tour, LPA observed one classroom with 25 napping children to one supervising staff and another classroom with 32 napping children and one supervising staff. Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 2 out of 6 staff. These staff were missing their MMR (measles) and Tdap (pertussis). This poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the facility did not comply with the section cited above in 3 out of 6 staff. 3 staff did not present proof of TB results. This poses/posed a potential health, safety or personal rights risk to persons in care.
